This is a new (in 2000) coverpage form the Chinese Embassy wishes
visa applicants to include when submitting a visa application.
(Most users may choose to save only the white page (graphic)
by clicking within the area of the white page and
selecting "save graphic" in the resulting pop-up menu.)
